De Anza - Steven & Lola Kaider Health Care Scholarship Program at De Anza College

Philanthropists Steven & Lola Kaider established a foundation to assist women with educational expenses as they returned to school to complete programs and serve in health care careers. This endowment to the Foothill-De Anza Foundation funds scholarships for these re-entry women. The Health Care A. The applicant must meet one or more of the following definitions of “reentry.”
1. The applicant did not attend college after completing high school for a period of one academic year or more (due to work, family, or personal circumstances).
2. The applicant attended college after high school but had to stop for a period of one academic year or more (due to work, family, or personal circumstances).
3. The applicant is currently trained, as a health care professional but due to an adverse employment situation caused by changing job markets must retrain.

B. The applicant must be accepted to or currently enrolled in any of the qualifying health care related programs offered at De Anza College:
Business Office Clerk, Insurance and Coding, Lab Assisting, Medical Assisting, Medical File Clerk, Medical Laboratory Technician, Medical Reception, Medical Records Clerk, Medical Secretary, Medical Transcription, Phlebotomist, LVN to RN Program, Registered Nursing Program.

C. The applicant enrolled in a qualifying health related program must pass each course in the program curriculum with a “C” or better, and remain in good standing in the program. The minimum course enrollment is six (6) units per quarter for the health care program courses.

D. Applicant must be enrolled in six (6) units or more of the core medical program in which he/she is enrolled and at the time of disbursement.

Comment on organization, volunteerism, and community activities in which you particaipated including when and the length of time involved in such activities. 5. Include any unique qualifications or special qualities that may set you apart from other applicants.
