Foothill & De Anza Santa Clara Family Health Plan Scholarship for Allied Health Students

Purpose: In honor of Santa Clara Family Health Plan’s (SCFHP) 25th anniversary, they have generously given funds for Foothill-De Anza students who are majoring in allied health care programs. SCFHP’s financial support is intended to empower the next generation of health care professionals who reflect the diversity of SCFHP members and community in Santa Clara County. Scholarships will be in the amount of $5,000 for full-time students (12 units or more) and $2,500 for part-time students (less than 12 units) for students who are SCFHP members.

• Applicants must be enrolled in allied health career training program at Foothill or De Anza, including the Community Health Worker certificate at Foothill. Acceptable programs include: Nursing, Medical Lab Technician, Dental Assisting, Dental Hygiene, Diagnostic Medical Sonography, Emergency Medical Services, Emergency Medical Technician, Paramedic, Pharmacy Technology, Radiologic Technology, or Respiratory Therapy.
• Applicants must be a Santa Clara Family Health Plan Medi-Cal member. (Application will have a supplemental question in which students attest that they are a member.)
• Students are welcome to re-apply and receive the SCFHP Scholarship two years in a row.

Note up to $20,000 is available. The number of scholarships given out will be determined by how many successful full-time and part-time students are in the applicant pool.
